Abstract
Obesity has become a global healthcare issue of the ХХІ century, being an integral component of the metabolic disturbances that constitute the metabolic syndrome (MS). The vast majority of individuals in this category experience aesthetic deformities of the anterior abdominal wall and are dissatisfied with both their health status and physical appearance. As a result, they primarily seek assistance from plastic surgeons for abdominal wall correction, often overlooking the underlying cause of their condition — excess body weight and metabolic disorders — which may negatively affect treatment outcomes.
Modern conservative approaches to the treatment of obesity and MS, such as diet therapy, lifestyle modification, and pharmacological treatment, demonstrate favorable short-term results. However, in the long-term — particularly among patients with significant excess body weight and severe metabolic disorders — their effectiveness may be insufficient.
Bariatric procedures — currently referred to as metabolic surgeries — such as sleeve gastrectomy and Roux-en-Y gastric bypass, are considered the most effective methods for treating obesity and MS, underscoring their significance in addressing this issue.
Thus, only after implementing conservative or surgical weight-loss interventions and achieving satisfactory results in body weight reduction and improvement or compensation of metabolic syndrome, should patients with aesthetic deformities of the anterior abdominal wall — caused by excess skin and altered abdominal contour — be considered for aesthetic surgical correction, namely abdominoplasty.
Therefore, a comprehensive approach to treating patients with anterior abdominal wall deformities and concurrent MS involves initially pursuing conservative or surgical weight-loss strategies and achieving metabolic control or improvement, followed by plastic and reconstructive surgery. This strategy ensures minimal risk of postoperative complications after abdominoplasty, maximal efficacy and safety, ultimately leading to significant improvements in patients’ quality of life, physical well-being, and psychological state.
